Birth Records in Florida
 
The Florida Department of Health�s State Office of Vital Statistics maintains birth records from 1917 on forward with some records dating back even further, all the way to 1865. The Office of Vital Statistics can assist eligible people with obtaining a copy of a birth certificate for those who were born in Florida. Birth records from 1902 to the present are not considered public record and requesters to a birth certificate in this time frame must meet eligibility requirements. Proof of identity is required in the form of a Florida driver�s license, Florida State ID card, U.S. Passport, or a military ID card.

Birth records over 100 years old become public records.
 
In order to obtain a certified copy of a Florida birth certificate less than 100 years old, the requester must be either:
- The person named on the birth certificate who is over 18 years old
- The parent, legal guardian or legal representative of the person listed on the birth certificate
- A person holding a court order
 
To apply for a certified copy of a Florida birth certificate, you must submit a request in writing. You should provide the following information about the person named on the birth certificate:
�         The full name of the person at the time of birth as well as any subsequent name changes
�         Date of birth
�         City or county of birth
�         The mother�s full name including her maiden name
�         The father�s full name
 
In addition, you should list information about yourself such as:
�         Your full name
�         Your relationship to the person listed on the birth certificate
�         Your daytime phone number
�         Your mailing address
�         Important: provide a copy of your valid picture ID. You must sign the copy of your ID and list the nature of your relationship to the registrant. If you are a legal guardian, you must also include notarized guardianship documents. Make sure the copies are clear and legible.

By Fax:
 
Ordering by fax is done through the VitalChek automated network. You will incur additional rush fees ($10) along with various VitalChek service and shipping fees. Call (877) 550-7428 to contact VitalChek. Alternately, you can check their website at www.vitalchek.com for electronic service. Again, additional rush, service and shipping fees apply.
 
Fees
 
Fees for a Florida birth certificate vary. Computer certification costs $9. This is a computerized copy with the embossed seal of Florida and is accepted as legal proof of citizenship and birth by all federal and state agencies. This certification is available for births from 1930 to present.
 
Photocopies of original birth certificates with the embossed seal of Florida cost $14 and are available for births from 1917 to present.
 
Additional copies (when ordered at the same time) cost $4 each.
 
Signed commemorative copies are available for $34.
 
Apostille or Exemplified Certification is a total of $39 with $29 being payable to the Office of Vital Statistics and $10 to the Secretary of State. No credit cards accepted for these types of certification.
 
Checks shall be made payable to: Vital Statistics and require a photo ID. Credit card orders will be processed through VitalChek and will incur additional fees.
 
Rush and express service and mailing is available. Standard mail is free and takes about 7-10 business days to process and mail your certificates. Additional time for the US Postal Service to deliver is required.
 
Add $10 for rush service and mark your request with �RUSH�. This will take 3-5 business days plus mail time.
 
$24.40 creates an express delivery. 1-2 business days are required to process the order and send via US Postal Express Overnight Service. The $24.40 includes the $10 rush fee as well as the $14.40 postal fee. A person must be available to sign for the package.
