Birth Records in Indiana
 
The Indiana State Department of Health maintains birth records for births occurring in Indiana from October 1907 to the present. For births that occurred before October 1907 you must search in the local health department in the county where the birth occurred.
 
When requesting a certified copy of an Indiana birth record, you must show proper identification. You may apply for a certified copy of a birth certificate in person, by mail, by fax or online.
 
You must be eligible to receive a copy and proof of your identity is required. In order to be eligible you must be:

• The person named on the record and over 18 years of age.
   
• The person named on the record who is under eighteen must provide signature, ID and the phone number of his or her parent or legal guardian.
   
• Mother or father of the person named on the record. The father must be named on the certificate and either married to the mother or have his paternity established.
   
• Grandparents, aunts, and uncles of the person named on the record but only if the parents of the person were married at the time of the birth.
   
• Legal guardians of the person named on the record
   
• Siblings of the person named on the record so long as both siblings are 21 years old or older.
   
• Spouse of the person named on the record.
   
• Children or grandchildren of the person named on the record as long as they are 21 years old or older.

The office accepts walk in requests between 10:00 AM and 2:00 PM EST Monday through Friday (They are closed on holidays). You will need to fill out an application, pay the proper fees and prove your identity. You will not walk out with a certified copy at this time. In general, it takes between three and four weeks to process your order.

How to Apply by Mail:
 
You may apply by mail by downloading and filling out the application listed above and sending it, along with proper payment, and a legible copy of your photo ID to:

How to Apply by Fax:
 
Using the fax to apply for a birth record is done through Vital Chek. Simply fax your completed application, photocopy of your picture ID and credit card information to 1-866-559-9629. You will incur additional service fees from Vital Chek. Vital Check is a service that takes the order and processes payment. Once the information is obtained, your eligibility verified and your payment processed, they forward the request to the State of Indiana who then retrieves the birth record and mails it to you.
 
How to Apply Online:
 
You can apply online using the Vital Chek website located at www.vitalchek.com.  When you use this service, you may pay your fees with a credit card however you will incur additional service fees and possible expedited delivery fees.
 
Fees:
 
The fee for birth record searches is set by law and costs $10. This is a non-refundable fee. If the record is found, a certified copy of the birth record is included in the search fee. If not, the fee is kept as a search fee. Additional copies, if ordered at the same time, cost $4 each. Amendments to a birth record cost $8. Payment can be in the form of a check or money order made out to the Indiana State Department of Health.
 
When ordering copies of birth records from the State of Indiana, you may request standard size or wallet sized copies.
