In New Mexico, birth records are available from the New Mexico State Health Department. In New Mexico, birth records are considered restricted access records. This means that only a few people will be allowed to receive copies of them.

Individuals who are permitted to request a copy of a birth certificate in New Mexico:

� Legal representative of the individual named on the birth certificate. Additional information may be required from legal representatives.
� Immediate family members including mother, father, sibling, child, maternal grandparent, paternal grandparent, or current spouse. Paternal grandparents can only request a copy of a birth record in New Mexico if the father is listed on the original birth certificate.
� Applicant must be at least eighteen years of age or must have a letter of release from a parent.

Basic information to be included with requests for birth certificates:

� Full name of the person named on the birth certificate.
� Date of birth. If the exact date of birth is unknown, an additional fee may be charged for retrieval of the information.
� County and city of birth.
� Hospital name and address where the birth occurred.
� Gender of the individual named on the birth certificate.
� Parents� names. Mother�s name must include her maiden name.
� Social security number of the individual named on the birth certificate.
� Relationship to the person whose name is on the birth certificate.
� Reason for the request for an official copy of the birth certificate.
� Applicant�s signature.
� Applicant�s daytime telephone number. This includes the area code.
� Applicant�s mailing address.
� Items for which you do not know the correct response should be filled in with the word �unknown.�

Fees associated with acquiring a copy of an official birth certificate from the New Mexico Department of Health and Hygiene:

� A $10.00 fee per copy for civilians. This fee covers the initial search and either one copy or a statement indicating that no record was found. Additional copies are $10.00 each.
� Mail orders must be paid by certified check or money order made payable to �New Mexico Vital Records.�
� Orders made in person can be paid with certified check or money order made payable to �New Mexico Vital Records.�
� Online orders are made through the Vital Chek Network Service and will be charged an additional $16.00 mailing and service charge.
� An additional $14.75 will be added to your cost for expedited service when using telephone, fax, or online ordering.
� Online, telephone, and fax requests must be paid with Visa, MasterCard, or American Express. The credit card must be in the name of the requestor.
� Incomplete requests will not be processed. They will be returned to the sender. This includes requests that are missing any of the following: payment, requested information about the individual named on the birth certificate, the requestor�s signature, and requested information about the requestor.

Five basic methods are available to obtain a copy of a birth certificate: postal mail, in person, telephone, fax, and online.

How to Apply for Certified Copies of Birth Certificates by Postal Mail

Send the completed form with signature, a legible copy of a government issued photo identification card, and payment to the following address:

A written request that includes the information listed above can be sent in lieu of a completed application form. Acceptable forms of identification include a state issued driver�s license or non-driver photo identification card that includes the current address. Photo identification should be enlarged by 150% or more. Requests are generally processed within three to four weeks.

How to Apply for Certified Copied of Birth Certificates in Person

Two different offices are available for walk in service to request a copy of a birth record. The individual making the request must complete an application form that is available at the office. Additionally, a legible copy of a government issued photo identification card is required. Acceptable forms of identification include a state issued driver�s license or non-driver photo identification card that includes the current address.

How to Apply for Certified Copied of Birth Certificates Online

To submit an online request, access the following link: http://www.vitalchek.com/homepage.asp

Next, you need to use drop down selection menus to fill in some of the information that pertains to your request including: type of certificate, city, county, date of the event, and the reason for the request. Select continue once the information has been filled in and you will be taken to a page to select your shipping options. Following this step, you will be required to complete the application.

How to Apply for Certified Copied of Birth Certificates by Fax

Send the completed form with signature, a legible copy of a government issued photo identification card, and credit card payment information to: 1-877-284-1066. Acceptable forms of identification include a state issued driver�s license or non-driver photo identification card that includes the current address. The copy of the birth record will not be faxed to the requestor, but it will be sent through the mail.
