Birth Records New York

In New York, birth records from 1910 to the present are available from the New York Department of Health and Mental Hygiene. Records prior to 1910 can be obtained by contacting the Municipal Archives at 311 or by visiting the following address: 31 Chambers Street, Room 103, New York City. Additionally, you can visit the Municipal Archives� website at: www.nyc.gov/html/records/

Individuals who are permitted to request a copy of a birth certificate:

� Legal representative of the individual named on the birth certificate.
� Immediate and extended family members including spouse, son, daughter, mother, father, brother, half brother, sister, half sister, grandparent/great grandparent (maternal or paternal), grandchild, great grandchild, stepparent, stepson, or stepdaughter. Additional information may be required from extended family members and legal representatives.
� Applicant must be at least eighteen years of age or must have a letter of release from a parent.

Basic information to be included with requests for birth certificates:

� Full name of the person named on the birth certificate.
� Date of birth. If the exact date of birth is unknown, an additional fee may be charged for retrieval of the information.
� County and city of birth.
� Hospital name and address where the birth occurred.
� Gender of the individual named on the birth certificate.
� Parents� names. Mother�s name must include her maiden name.
� Social security number of the individual named on the birth certificate.
� Relationship to the person whose name is on the birth certificate.
� Reason for the request for an official copy of the birth certificate.
� Applicant�s signature.
� Applicant�s daytime telephone number. This includes the area code.
� Applicant�s mailing address.
� Items for which you do not know the correct response should be filled in with the word �unknown.�

Fees associated with acquiring a copy of an official birth certificate from the New York Department of Health and Hygiene:

� A $15.00 fee per copy for civilians. This fee covers the initial search and either one copy or a statement indicating that no record was found.
� Mail orders must be paid by check or money order make payable to the New York Department of Health and Hygiene.
� Orders made in person can be paid with cash, check, or money order.
� Telephone, fax, and online orders will be charged an additional $5.50 mailing and service charge.
� If you use an express carrier, an additional $12.50 will be added to your cost.
� Fees at the local municipal offices can be obtained by contacting that office directly.

Five different methods are available to obtain a copy of a birth certificate: postal mail, in person, telephone, fax, and online.

How to Apply for Certified Copied of Birth Certificates by Telephone

Call the following telephone number: 1-212-788-4520. Follow the instructions provided to you in order to select the automated service or the live operator service. If you select the live operator service, the waiting time can be as long as 20 minutes. The automated service is available 24 hours a day, 7 days a week.

An individual can only use telephone ordering for requests for his or her own birth certificate or for his or her child�s birth certificate, provided the parent is named on the birth certificate. All other requests must be made in person or by mail. Fees apply as noted above.

How to Apply for Certified Copied of Birth Certificates by Fax

Complete the application form available at the following link:

Sign and date the form. Fax the application along with your identification requirements to the following number if you are calling from one of the five boroughs: 212-962-6105 or the following number if you are calling from outside New York City: 800-908-9146.

How to Apply for Certified Copied of Birth Certificates Online

To submit a request for a birth certificate in New York, access the following link: www.vitalchek.com/

Drop down selection menus are used. You need to fill in the following information that pertains to your request including: type of certificate, city, county, date of the event, and the reason for the request. Select continue once the information has been filled in and you will be taken to a page to select your shipping options. Following this step, you will be required to complete the application.

The certificate will only be mailed to the address that corresponds to the credit card
account that is used for payment. Requests are generally processed within three to five business days. Birth certificates will be sent through postal mail unless a request and payment has been made for an express carrier including UPS, FedEx, and Express Mail. A signature is required to receive a certified copy of the birth certificate through an express carrier. An additional charge is assessed for the use of an express carrier.

All payment must be made with a credit card and follows the guidelines included above. Online services are available 24 hours a day, every day of the year.

An individual can only use online ordering for requests for his or her own birth certificate or for his or her child�s birth certificate, provided the parent is named on the birth certificate. All other requests must be made in person or by mail. Fees apply as noted above.
